The undersigned hereby certifies the following:
  1. I have agreed to allow Genesis Credit Restoration to work on my behalf and collect documentation and personal information from me, and pass this information on to 950 Credit, Inc. for the purpose of rent reporting. This information includes, but is not limited to my date of birth, social security number, driver's license information, lease information, etc.
  2. I have agreed to allow 950 Credit Inc. (working in partnership with Genesis Credit Restoration) to act as a third party reporter between myself, as a renter, and the landlord/property management company.
  3. I will provide documentation containing various information for the purposes of the credit reporting. I certify that all of the information is true and complete. I made no misrepresentations in the documentation provided nor did I omit any pertinent information.
  4. I fully understand that it is a Federal crime punishable by fine or imprisonment, or both, to knowingly make any false statements when participating in this reporting process under the Federal law.
  5. You may notify Genesis Credit Restoration of your intent to cancel at any time by emailing them at Jean@genesiscreditrestoration.com. They will cancel your account with 950 Credit, Inc.
  6. I fully indemnify and hold harmless 950 Credit, Inc. in the event that any data breaches occur in systems that may or may not be under 950 Credit's control. This includes any data breaches as a result of use or misuse of these systems by personnel outside of 950 Credit's control and/or supervision, including those systems controlled by Genesis Credit Restoration.
  7. If there is a dispute regarding your payment of fees, or the Service by either you or Genesis Credit Restoration, then 950 Credit Inc shall have the right to terminate and or delete your account without prior notice. To reinstate the disputed account, the total disputed amount plus a dispute fee of $15.00 per dispute will need to be collected prior to reactivating/ reinstating the account. For the avoidance of doubt, if you purchase a monthly (or other periodic) membership, you agree and acknowledge that your subscription has an initial and recurring payment charge each month.
  8. I have reviewed and acknowledge the 950 Credit, Inc. Terms and Conditions (Click Here to View It) and Privacy Policy (Click Here to View It).
  9. I understand that neither Genesis Credit Restoration nor 950 Credit, Inc. has control over any increase in my credit score, and 950 Credit, Inc. only reports my rent payments to the credit bureaus. The credit bureaus are solely responsible for any increase in the credit score based on their calculations and algorithms.
  10. I understand that 950 Credit, Inc. may report information about my account to the credit bureaus; and that late payments, missed payments, or other defaults on my account may be reflected in my credit report.
  11. I agree to the electronic delivery of information and notices from 950 Credit, Inc. and agree to provide and continue to provide a valid email address for the purposes of delivery of electronic notices. This consent includes, without limitation, the electronic delivery of furnishing of negative information to a Credit Reporting Agency.
Expedited Service Policy
If Expedited Service has been requested, I understand that:
  1. The account will be reported to TransUnion and Equifax within 3 business days of receiving all required documents from you or Genesis Credit Restoration, and your Landlord or Property Management Company. After we report your account to TransUnion and Equifax, then these bureaus will have the new tradeline active within 3 business days.
  2. Expedited Service accounts do not qualify for refunds for any reason.
Authorization to Release Information
To Whom It May Concern:
  1. I have entered into a Tenant Service Agreement with 950 Credit Inc., in partnership with Genesis Credit Restoration. As part of the Agreement, Genesis Credit Restoration and/or 950 Credit Inc. may verify information contained in my rent verification process and in other documents required in connection with reporting throughout the time I am under contract as part of its quality control program. I further authorize my landlord (past & present) to provide Genesis Credit Restoration and/or 950 Credit, Inc. a copy of my Lease Agreement(s) and payment history.
  2. I understand and agree that Genesis Credit Restoration and/or 950 Credit Inc. reserves the right to change the rental history verification process to a require alternative documentation. This may include verifying the information provided on the application with the landlord/property management company and/or the banking financial institution.
  3. Genesis Credit Restoration and/or 950 Credit Inc. may address this authorization to any party named in the lease and or banking institution.
  4. A copy of this authorization may be accepted as an original.

  6.   9. Entiendo que ni Genesis Credit Restoration ni 950 Credit, Inc. tienen control sobre mi puntaje de crédito ni de que este suba, y que 950 Credit, Inc. solo reporta mis pagos de renta a los buros de crédito. Los buros de crédito son los únicos responsables por el aumento al puntaje de crédito basado en sus cálculos y algoritmos.
  10. Entiendo que 950 Credit, Inc. puede reportar información sobre mi cuenta a los burós de crédito; y que los pagos atrasados, pagos faltantes, u otras faltas pueden verse reflejados en mi reporte de crédito.
  11. Estoy de acuerdo en recibir información y notificaciones de 950 Credit, Inc. de manera electrónica, y estoy de acuerdo en proveer de manera continua una dirección de correo electrónico válida con el propósito de recibir notificaciones electrónicas. Política de Servicio Rápido / Express
Si se ha requerido el servicio rápido, entiendo que:
  1. La cuenta será reportada a TransUnion y Equifax 3 díasdespués de haber recibido todos los documentos necesarios por parte de usted, Genesis Credit Restoration o el arrendador. Después de reportar su cuenta a TransUnion y Equifax, ellos activaran la nueva línea de crédito dentro de 3 díashábiles.
  2. Cuentas con servicio rápido no califican para ningún tipo de reembolso por ninguna razón.
Autorización para Compartir Información
A quien corresponda:
  1. He entrado en un Acuerdo de servicios para inquilinos con 950 Credit Inc., en conjunto con Genesis Credit Restoration. Como parte del Acuerdo, Genesis Credit Restoration y/o 950 Credit Inc. pueden verificar la información contenida en mi proceso de verificación de alquiler y en otros documentos requeridos en relación con los reportes durante el tiempo que este bajo contrato como parte de su programa de control de calidad. Además, autorizo a mi arrendador (pasado y presente) a proporcionar a Genesis Credit Restoration y/o 950 Credit, Inc. una copia de mi(s) contrato(s) de arrendamiento e historial de pagos.
  2. Entiendo y acepto que Genesis Credit Restoration y/o 950 Credit Inc. se reservan el derecho de cambiar el proceso de verificación del historial de alquiler para requerir documentos alternos. Esto puede incluir la verificación de la información proporcionada en la solicitud con el arrendador/compañía de administración de la propiedad y/o la institución financiera bancaria.
  3. Genesis Credit Restoration y/o 950 Credit Inc. pueden dirigir esta autorización a cualquier parte nombrada en el contrato de arrendamiento o institución bancaria.
  4. Se puede aceptar una copia de esta autorización como original.
